Why certain cancer diagnoses are being investigated in ethylene oxide cases

Ethylene oxide is classified as a human carcinogen by the EPA, the National Toxicology Program, and the International Agency for Research on Cancer. Those classifications are not precautionary. They are based on decades of animal studies and long-term human occupational research showing increased cancer risk after prolonged EtO exposure.

EtO is a direct-acting alkylating agent, meaning it binds to DNA and causes mutations without requiring any metabolic activation. This mechanism is what makes the chemical effective at killing pathogens in medical sterilization, and it is the same mechanism that makes prolonged exposure dangerous for nearby residents. Because EtO damages DNA directly, blood-forming and lymphatic tissues are particularly vulnerable, which is why the cancers most consistently observed in exposed populations involve the blood and lymph system.

Lymphoma and its subtypes

One of the most consistently studied associations in EtO research involves lymphohematopoietic cancers, a category that includes both Hodgkin lymphoma and non-Hodgkin lymphoma. The EPA's carcinogen assessment for ethylene oxide specifically identifies elevated risks of lymphoid cancers in occupationally exposed populations, particularly workers in sterilization and chemical manufacturing facilities. Non-Hodgkin lymphoma subtypes that appear in occupational studies include diffuse large B-cell lymphoma, follicular lymphoma, mantle cell lymphoma, and marginal zone lymphoma, among others.

Leukemia

Studies of occupational EtO exposure have reported increased risks of leukemia, including acute myeloid leukemia. The biological explanation is consistent with what researchers know about EtO's DNA damage mechanism. When alkylation affects blood-forming cells in the bone marrow, it can trigger mutations that lead to leukemia. The National Toxicology Program classifies EtO as known to be a human carcinogen based in part on this body of evidence from blood cancer research in exposed worker populations.

Breast cancer

Breast cancer has been examined in multiple occupational and environmental studies of EtO exposure. The EPA's risk model for ethylene oxide notes that long-term inhalation exposure is associated with increased cancer risk across multiple tissue types, including breast tissue. That connection is especially meaningful in community exposure research, where the focus is on prolonged lower-level exposure over many years rather than the concentrated workplace levels studied in industrial settings. A 2023 study in Environmental Health Perspectives found those with higher estimated residential EtO exposure had a higher risk of ductal carcinoma in situ, adding to a mounting body of research that links the chemical to breast cancer.

Why residential exposure near facilities is under review

EtO investigations are no longer limited to occupational settings. Because ethylene oxide is a gas that disperses into the surrounding air after release, residents living near emitting facilities may be exposed through long-term inhalation of contaminated outdoor air. The EPA's National Air Toxics Assessment evaluates cancer risk from air toxics, including EtO, based on long-term modeled exposure across geographic areas rather than isolated incidents.

For this reason, legal and public health reviews often focus on residents who live within approximately three miles of an EtO-emitting facility, particularly those with long-term residence of 10 years or more, childhood to adulthood exposure patterns, and proximity to homes, schools, and community institutions within the surrounding area.

Why only certain cancers are included in these investigations

Not every cancer is equally associated with EtO exposure. The diagnoses most commonly reviewed share three characteristics: stronger epidemiological evidence from occupational studies, biological plausibility based on EtO's DNA damage mechanism, and observed increases in long-term exposed populations. The cancers that consistently meet those criteria are lymphoma, leukemia, breast cancer, and other lymphohematopoietic cancers.
